This
section lists services that are designed to improve the health and wellbeing of people
using the services. It includes information about Intermediate Care
Services which are designed to prevent avoidable admissions or facilitate a
speedier discharge from hospital.
Entries marked with an asterisk are awaiting the final restructure of health services in Wales and will be updated as soon as their statements of purpose become available.
