Health


This section lists services that are designed to improve the health and wellbeing of people using the services.  It includes information about Intermediate Care Services which are designed to prevent avoidable admissions or facilitate a speedier discharge from hospital.  

Entries marked with an asterisk are awaiting the final restructure of health services in Wales and will be updated as soon as their statements of purpose become available.

Accident and Emergency (North Glamorgan NHS Trust)*

Provision of Emergency care and treatment for people sustaining an accident or an emergency health related condition.  

Acute Adult Psychiatric (Pontypridd and Rhondda NHS Trust)*

Provides secondary care for patients over 17 years of age suffering from acute mental health problems.

Acute Medical Unit (Pontypridd and Rhondda NHS Trust)*  Provides secondary care acute care physician assessment for emergency medical admissions to direct patients in Rhondda and Taff Ely accordingly.

Age Concern Community Outreach Team (2009)

Offers support to people (60+) irrelevant of their illness or disability as well as specialise in people suffering from dementia.

Cardiac Rehabilitation Department (Pontypridd and Rhondda NHS Trust)* Provides evidenced based, patient focussed care to those patients requiring cardiac rehabilitation services.

Cardiac Rehabilitation Scheme RCT (2009)

Provides advice, support and a carefully structured activity based scheme for people living in Rhondda Cynon Taf who have suffered a coronary incident

Catering services (2009) Delivers a daily service to people who would otherwise have difficulty in making or arranging a nutritious and well balanced cooked meal
Community Addictions Unit (2009) An inpatient detox service for drug and alcohol misusers in Merthyr Tydfil and Rhondda Cynon Taf  
Community Drug and Alcohol Team (Pontypridd and Rhondda NHS Trust)*  The CDAT is predominantly an out patient service whose main aim is to treat people with problems related to substance misuse. 
Community Gynaecology Services (North Glamorgan NHS Trust)* Provides specialist services to a wide variety of clients on an individual basis to meet needs within Merthyr Tydfil.  
Community Macmillan Clinical Nurse Specialists (North Glamorgan NHS Trust)*  To provide a service to patients and their carers with a diagnosis of malignant disease or specific life threatening illness.

Older Person's Mental Health and Dementia Services -  RCT (2009)

Outreach and day care based services for people with dementia (of any age) or mental health issues (usually 65+) living in the community.

Assessing and arranging care for people who have memory problems/dementia (regardless of age) and those over 65 who have a diagnosis of a functional mental illness.

Assessing and arranging care for people who have memory problems/dementia (regardless of age) and those over 65 who have a diagnosis of a functional mental illness.

Orthoptic Service (North Glamorgan NHS Trust)* Provide a service for people with ocular motility problems living in Merthyr Tydfil and Cynon.  
Orthotics Department (North Glamorgan NHS Trust)* An orthotic service provided from Prince Charles, Aberdare and Mountain Ash hospitals for Merthyr Tydfil and Cynon residents

Palliative Care Services (North Glamorgan NHS Trust)*

Provides specialist palliative care to patients and families with Prince Charles, St Tydfil’s, Mountain Ash and Aberdare hospitals.  

Palliative Care Services (Pontypridd and Rhondda NHS Trust)* Provides comprehensive palliative care services, including in-patient beds, day centres, community palliative care nursing, hospital support teams and outpatient services in Rhondda and Taff Ely.  
Parkinson's Disease Society Wales (2009) An information, support and advice service for Parkinson Disease sufferers.
Physiotherapy (North Glamorgan NHS Trust)* An acute in and out patient service for adults.  The service is offered at Prince Charles, St. Tydfils, Aberdare and Mountain Ash hospitals.
