Assessment
What is Unified assessment?
Unified Assessment is a process where an holistic approach is taken to assess a
person's needs. The result of an assessment carried out under Unified Assessment will
lead to more effective joint working and prevent serial assessments by different
agencies.
Benefits should accrue for patient and service users (and their carers) in
terms of a more integrated approach across all care agencies, to delivering care packages that
promote the independence of the patient or service user.
The development of a range of Intermediate Care Services will give us greater opportunity to use the Unified Assessment Process, to work collaboratively and streamline the process of accessing support at the right time and in the right place for the maximum benefit to our Service Users.
Guidance is available to maintain best practice when working either as a
single agency or when working jointly with other care agencies to record, maintain and store
assessment information and care plans in a Unified Assessment summary record.
The Guidance provides a framework to develop single eligibility criteria to ensure there is fair access to services.
